Nuvance Health has a network of convenient hospital and outpatient locations — Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York — plus multiple primary and specialty care physician practices locations, including The Heart Center, a leading provider of cardiology care, and two urgent care offices. Non-acute care is offered through various affiliates, including the Thompson House for rehabilitation and skilled nursing services, and the Home Care organizations.

JOB SUMMARY: Provides educational services to diabetic patients and their families to promote self care and independence in diabetes management. Educational services include conducting a complete assessment of learning needs, developing a teaching plan, providing appropriate educational experiences, and evaluating effectiveness of education.

MINIMUM QUALIFICATIONS:

Education: Graduate from an accredited nursing program.

Licensure/Registration/Certification:

Currently licensed as a Registered Professional Nurse in New York State.

Currently certified as a Diabetic Educator by the American Association of Diabetic Educators.

Work Experience:

Competent as a nurse practitioner, minimum five years clinical experience.

Physical Requirements:.

May be on feet most of the day. Some lifting, stretching and other physical exertion, moving equipment and teaching. Often works additional shifts, as needed.

Work Environment:

May be exposed to patient’s conditions and some unpleasant sights, smells and contagious diseases. Required to attend off site functions.

Job Relationships (Responsible to/for, works with):

Responsible to the Director of Education and Training.

Works with Health Educator, and other members of the healthcare team.
