Summary:

Our Physical Therapist’s provide services that identify, prevent, remediate and rehabilitate acute or prolonged physical dysfunction or pain. Our PT’s evaluate and analyzes patients and therapeutic application, physical and chemical agents, exercise, instruction to patient/family and other procedures.

Responsibilities:

• Performs physical therapy assessment including functional limitations, rooms, capabilities, mobility status, endurance status, ADL status.
• Identifies patient and family needs for other home health services and refers for same when needed.
• Provides services that are ordered by the physician as indicated in the plan of care. Communicates with the physician(s) involved in the plan of care and other health care providers as appropriate.
• Provides appropriate treatment. Treats patients to relieve pain, develop and restore function and regain the optimum level of independence using appropriate equipment and modalities.
• Records on a visit basis, all evaluation data, treatments and client's response to therapy, provide client's discharge planning.
• Participates in case management conferences with team members to coordinate physical therapy and a total plan of care.
• Provides counseling and education to patients, family, and other health care personnel in the patient’s treatment regimen, and documents to whom this instruction has been given.
• Provides or arranges for equipment required to increase the client's function and independence.
• Participates in the agency quality assurance and performance improvement program and the agency-sponsored in-service training.
• Supervises care provided by the PTA(s) and home health aides as appropriate.
• Fulfills all compliance responsibilities related to the position.

 

Education and License Requirements :

• Current State of Connecticut Physical Therapy Licensure, graduate of an approved school of Physical Therapy or graduate of accredited college or university with a certificate in Physical Therapy.
• 3 or more years of previous experience in all aspects of physical therapy *preferable* but 1 years of experience is accepted
• Current BCLS certification.
