Nuvance Health has a network of convenient hospital and outpatient locations — Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York — plus multiple primary and specialty care physician practices locations, including The Heart Center, a leading provider of cardiology care, and two urgent care offices. Non-acute care is offered through various affiliates, including the Thompson House for rehabilitation and skilled nursing services, and the Home Care organizations.
SUMMARY: In collaboration with Nurse Case Managers, the social worker provides patient/family interventions to support clinical, satisfaction, financial and compliance related outcomes. He/she paces the case from a psychosocial-economic perspective by assisting patients and families in adjustment to episodes of acute and chronic illness; in coping with lifestyle changes precipitated by illness or disability; in participating in restorative health programs; in carrying out discharge recommendations; and in addressing issues of end of life care. Using a variety of treatment modalities, the social worker assists patients and families in the alleviation or resolution of social, financial and emotional problems related to illness, health care and rehabilitation.
RESPONSIBILTIES AND STANDARD
1. Completes an assessment of patient needs and family supports, evaluates available data, selects appropriate social work interventional methods and develops, implements and evaluates plans in conjunction with the Nurse Case Manager and multidisciplinary team.
2. Works with the Nurse Case Manager, patient and family to develop, implement and evaluate the effectiveness of discharge plans, in collaboration with the multidisciplinary team and community agents.
3. Provides support and counseling to patients and families experiencing and/or anticipating issues to adjusting to illness, catastrophic diagnoses, changes in living situations or loss/grief.
4. Takes responsibility for adhering to established departmental policies, procedures and objectives and for professional development.
JOB QUALIFICATIONS
Education - Masters degree in social work
Licensure – Current, NYS license Preferred – CCM certification Experience
Preferred – 1 year in hospital setting

Ability to form positive, collaborative relationships with interdisciplinary staff, patients and families
Ability to negotiate effectively with internal and external providers
Ability to manage multiple priorities -Analytical, problem-solving abilities
Basic knowledge of computers and clinical applications
