Nuvance Health has a network of convenient hospital and outpatient locations — Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York — plus multiple primary and specialty care physician practices locations, including The Heart Center, a leading provider of cardiology care, and two urgent care offices. Non-acute care is offered through various affiliates, including the Thompson House for rehabilitation and skilled nursing services, and the Home Care organizations.


Job Summary:

Provides oversight and coordination for transcription services for the hospital. Transcribes, proofreads and edits complex technical, medical and dental reports for a broad range of clinical specialties and departments to document patient care using machine dictation and word processing equipment. Accountable for the accurate transcription of dictated medical reports, H&Ps, etc. Performs secretarial duties as needed.

 

Regulatory Requirements:

High School graduate or equivalent.

Experience as a medical transcriptionist and/or E&M coding preferred.

Previous hospital or medical office medical record experience preferred.

Responsibilities:

1.Responsible for the oversight of all physician/PA/therapist dictation: H&Ps, operative notes, therapeutic procedures, consultations, discharge summaries, rehab assessments, etc.

2.Acts as the organizational liaison in communications with contracted transcription services

3.Proofreads transcribed documents for accuracy and completeness; verifies patient information for accuracy and completeness, as needed.

4.Randomly reviews transcribed documents for accuracy of terminology, punctuation, grammatical, typographical errors and misinterpretations, and notifies transcription service as necessary.

5.Identifies inconsistencies, and, when necessary, seeks clarification from the dictating individual

6.Update provider lists, report types and formats with transcription service.

7.Identifies priority and STAT reports not so identified by dictator and notifies transcription service.

8.Monitors, evaluates, and makes recommendations for continuous quality improvement

9.Copies and distributes documents as required

10.Maintains daily log of dictated and transcribed reports and provides HIM Director with daily and monthly documentation.

11.Understands medicolegal implications and responsibilities related to the transcription of patient medical records.

12.Reviews ED records and enters procedures into the Emergency Department/LYNX/E- point application prior to the coders coding the records.

13.Scans medical records to current vendor in accordance with compliance policy for coding auditing

14.Utilizes medical dictionaries, current Physician’s Desk Reference, Hospital Formulary and other reference materials as needed.

15.Answers the phone in a professional and courteous manner; takes messages; routes calls to appropriate staff members.

16.Performs clerical work as needed: filing, pulling patient charts etc.

17.Maintains a good working relationship within the department and with other departments and physicians.

18.Uses the principles of AIDET in every customer encounter.

19.Performs other duties as requested.

 

Language Skills:

Excellent skills in English usage, grammar, punctuation, and style.

Excellent written and oral communication skills. Additional languages preferred.

 

Skills:

·Basic computer knowledge, and ability to operate word processing equipment, and other equipment as specified, and to troubleshoot as necessary.

·Knowledge of medical terminology, anatomy and physiology, disease processes, signs and symptoms, medications, and laboratory values related to a specialty or specialties.

·Knowledge of medical transcription guidelines and practices.

·Excellent organizational skills.

·Excellent listening skills including the ability to hear and understand dialects and various accents.

·Ability to use a vast array of professional reference materials.

·Ability to work independently with minimal or no supervision.

·Ability to work under pressure with time constraints.

·Ability to understand and apply relevant legal concepts (e.g, confidentiality).

·Proven business skills (scheduling work, client relations).

Work Shift: Shift with be Monday to Friday from 7:30to 4:00 PM.
