Nuvance Health has a network of convenient hospital and outpatient locations — Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York — plus multiple primary and specialty care physician practices locations, including The Heart Center, a leading provider of cardiology care, and two urgent care offices. Non-acute care is offered through various affiliates, including the Thompson House for rehabilitation and skilled nursing services, and the Home Care organizations.


Summary:

Under the direction of the Director of Pharmacy, the pharmacy technician assists in the preparation and distribution of medications and maintains drug inventory and patient records. Pharmacy technicians work under the supervision of a registered pharmacist in providing safe and effective medication management for the patients.

 

Responsibilities:

1.Prepares medication in time for next delivery or immediately if a STAT order as per policy.

2.Organizes medications to be checked by the pharmacist on duty and delivered at the time of the next round.

3.Restocks IV room and clean room with appropriate medications and supplies.

4.Documents refrigerator temperatures & outcome of any refrigerators previously out of bounds.

5.Triages incoming telephone calls and pharmacy window inquiries, and transfers medication-related questions to the pharmacist when appropriate.

6.Performs basic Pyxis® functions, including loading and unloading medications, print basic reports, and troubleshooting.

7.Refills Pyxis® med stations in a compliant manner.

8.Completes nursing units inspections in an accurate and timely manner when assigned.

9.Maintain and Model Nuvance Health Values.

10.Demonstrates regular, reliable and predictable attendance.

11.Performs other duties as required.

A Pharmacy Tech may focus on medical reconciliation and if so the responsibilities for are replaced with:

Responsibilities I

·Assumes responsibility for the development of an accurate medication list for all patients in the ED; both those admitted and those not being admitted (especially level 1, 2, 3).

·Develops a list of current medications utilizing all available resources.

·Compares list with previous discharge reconciliation (when available).

·Clarifies all medication changes and last dosages with patient, family, and/or facility MAR.

·Verified all food, drug, and environmental allergies by the same sources.

·Makes necessary changes to medication list, enters, them accurately in the EMR resolving all discrepancies.

·Communicates any major changes to primary RN or attending physician

·Assists in the delivery of patient safety by utilizing an accurate medication list, application of pharmacology knowledge, and critical thinking skills

·Demonstrates knowledge of therapeutic actions, side effects and interactions of medications.

·Expedites medication reconciliation entry of the most critical patients by communicating effectively with ER physicians and nurses or utilizing yield icon on FirstNet.


Other information:

High School Diploma or equivalent
Minimum of six months to one year in hospital pharmacy or related job preferred
Required to pass the Pharmacy Technician Certification Board (PTCB) within six months of employment. After passing PTCB exam submission for NYS licensure is required within 30 days.
Communicate clearly in-person / over the phone / respond to questions

Ability to comprehend information from electronic sources or computer screens
