Nuvance Health has a network of convenient hospital and outpatient locations — Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York — plus multiple primary and specialty care physician practices locations, including The Heart Center, a leading provider of cardiology care, and two urgent care offices. Non-acute care is offered through various affiliates, including the Thompson House for rehabilitation and skilled nursing services, and the Home Care organizations.


SUMMARY:

Performs, under the direct supervision of a registered nurse, specific clerical functions and non-professional services necessary to maintain efficient and effective operations of the nursing unit or department consistent with the philosophy, goals, policies and procedures of the nursing department and hospital administration.
Responsibilities:

Coordinates all activities involved in the preparation and maintenance of the patient record to ensure accuracy and completeness. Assembles, as needed, all admission records including patient record, addressograph plate and I.D. band.
Transcribes physician’s orders onto appropriate forms and records in accordance with departmental policies.
Maintains on a daily basis patient’s clinical records including graphic sheet, Intake and Output, MAR’s and others as needed in an accurate and timely manner.
Files test results and patient reports as necessary.
Reduces and stores patient records for patients with extended stay or voluminous records.
Prepares patient record for discharge and/or transfer including copying, chart breakdown and contacting appropriate departments.
Ensures that accurate and timely communication is facilitated and that the nursing unit/department is presented in a positive manner.
Answers telephone within three rings in a courteous manner, identifying self and nursing unit at all times.
Maintains complete and accurate telephone messages for all members of the department, for patients and for physicians assuring accurate routing of messages.
Places calls to physicians and to other departments for the coordination of patient care, as needed.
Greets nursing department staff, physicians, patients and visitors promptly and courteously.
