Physical Therapist 2 Outpatient

Full-time 40 hours

Day shift- no weekends, Mon-Fri, 7am - 8pm 2 shifts 1130am to 8pm

Summary:
Attention certified Physical Therapists! Main Street Rehabilitation in Danbury CT is a dynamic outpatient rehabilitation facility with Nuvance Health and we are currently looking for a Full time Physical Therapist to join our outpatient team of experienced and skilled clinicians. Our facility offers PT, OT, ST and aquatic services to both the orthopedic and neurologic population. We are looking for a PT with outpatient experience or the desire to work in outpatient, to work with our team of therapists to implement specific treatment programs and collaborate with all disciplines to provide quality PT services to our community.

Required:

• CT Licensed Physical Therapist 
• Bachelor’s degree, prefer Doctorate
• CPR
• Outpatient Office, Orthopedic, Neurologic, diagnoses including vestibular

Responsibilities:

• Performs therapy evaluations, treatment planning and implementation utilizing department clinical guidelines & protocols. Participates in team conference /rounds for purposes of coordination of patients plan of care
• Provides patient/ family education and training.
• Initiates discharge planning involving patient, family and team members as appropriate. Recommends equipment to meet patient needs.
• Attends 20 hours of continuing education per year. Provides 2 hours of in-service education per year to staff.
• Provides full-time clinical experience to students.
• Acts as mentor to lower level staff.
• Participates in orienting, teaching, guiding and evaluation of assistive personnel. Acts as mentor to lower level staff. Delegates to support personnel.

Salary Range:

$40.00-$62.00 Hourly depending on experience
