Summary:

Uses the therapeutic application of physical and chemical agents, exercise, and other procedures for the treatment and prevention of illness and injury; evaluates patient’s abilities and needs; establishes plan of care and goals. Objective: achieve the highest possible level of functional independence for each patient - restore function and decrease potential for further disability. Population served based on scope of services in the department.

 

Responsibilities:

Thoroughly evaluates and sets plan of care to meet patients identified treatment needs.

Establishes objective, measurable and function patient goals.

Implements and updates treatment program to maximal effectiveness.

Effectively oversees patient care delivery by assistants/aides.

Completes all documentation within established time frames and according to regulatory guidelines.

Involves patient/family in establishing treatment plan and accommodates for individual needs (cultural differences, language, etc.)

Participates in educational in-services, meetings and performance improvement activities as requested by Coordinator.

Assists in the training/supervision of affiliating students/volunteer as requested.

Maintains all department equipment and treatment areas in a clean and safe manner.

Accurately enters patient charges/provides patient billing information to office staff.

Maintain and Model REACH Values (Respect, Excellence, Accountability, Compassion, Honor)

Demonstrates regular, reliable and predictable attendance

Performs other duties as required

Other information:

Education: Graduate of an accredited curriculum with a minimum of a Doctorate of Physical Therapy. For individuals who completed appropriate Physical Therapy education prior to 2015, with a valid Physical Therapy license in the state of practice or proof of license eligibility, a Bachelor’s degree or greater in Physical Therapy will be accepted in lieu of the Doctorate of Physical Therapy degree.

Current license to practice Physical Therapy : Successful completion of minimum educational requirements for licensure including all clinical practice and comprehensive examinations.

Current CPR Certification Meets continuing education requirements for state Licensure & CPR Certification
