Position Summary

We are seeking a Speech Therapist to assess and evaluate speech, language, and hearing disorders. The therapist will develop individualized care plans in collaboration with the patient, caregivers, and the broader home health team, and provide recommendations for corrective treatment to the physician. This role also includes ongoing patient assessment, documentation, family education, and participation in the interdisciplinary care process.

Key Responsibilities

1. Conduct diagnostic evaluations to assess speech, language, and hearing disorders and recommend treatment to the physician.
2. Develop and participate in the total care plan and case management in collaboration with the physician and care team.
3. Maintain detailed records of visits, evaluations, care plans, therapy progress, and discharge summaries.
4. Participate in case management conferences to coordinate therapy with the overall plan of care.
5. Report client progress, reactions to treatment, and changes in condition to the physician; reassess at least every 60 days.
6. Instruct and educate patients, families, and team members on methods to improve speech, language, and swallowing disorders.
7. Provide written instructions for care and therapy to the home health team, leaving copies in the patient’s home and record.
8. Participate in quality assurance and performance improvement initiatives.
9. Supervise home health aide care as appropriate.
10. Complete all compliance responsibilities, including attending in-service training.
11. Perform other duties as assigned.

Education & Certification

• Required: Master’s degree in Speech Therapy from an accredited program.
• Certified by the American Speech-Language-Hearing Association (ASHA); successful completion of the National Registration Examination.
• For international graduates, completion of the ASHA-prescribed registration examination.
• Supervisors must have a minimum of 3 years clinical experience and be licensed in Connecticut.
• Recommended: At least 1 year of full-time experience in hospital, clinic, or agency under supervision; refresher courses for those inactive for 5+ years.

Experience & Skills

• Minimum: 1 year clinical experience in speech therapy.
• Desired:
• Functional Independent Measurement (FIM) certification
• CPR certification
• Experience with FEES (Fiberoptic Endoscopic Evaluation of Swallowing) or MBS (Modified Barium Swallow)

Working Conditions

• Manual Skills: Requires motor coordination and finger dexterity.
